First name
Middle name (if applicable)
Last name (required).
Suffix, such as Jr., Sr., or III (if applicable)
Enter a preferred name (if applicable).
Enter your date of birth (required)

Click Next to advance to the final Credentials section.
Credentials Section
The final step in the Create Profile section is to set up a secure Password that meets the security criteria requirements.
Your A secure password must meet the following requirements:
be at least 8 characters in length
contain at least one uppercase letter
contain at least one lowercase letter
contain at least one number
contain at least one of the following special characters ( !, @, #, $, %, ^, &, or *)
must NOT contain your name
